OnSite Medical House Calls is a home-based medical practice providing Primary Care, Palliative Care, Psychiatric Services, and Chronic Care Management to patients in the comfort of their home or assisted living facility throughout Maryland.
We primarily care for chronically ill, medically complex, homebound, or mobility-limited patients who have difficulty getting to a traditional doctor’s office.
We visit private residences, assisted living facilities, and skilled nursing facilities.
We service many areas throughout Maryland. Contact our office to confirm whether your zip code is within our coverage area.
We accept Medicare, Maryland Medicaid, and many commercial insurance plans. Insurance coverage can vary, so we recommend contacting our office to verify eligibility
Most patients are seen approximately every 4 to 6 weeks depending on their medical needs and condition. If you are seen by both our primary care and palliative care providers, it can be a little more frequently.
Availability varies based on provider schedules and geographic routing. Urgent needs are prioritized whenever possible.
You will be scheduled based of zip code and when your provider is in your area. Due to the nature of house calls and the unpredictability of patient care needs, we are unable to provide exact appointment times. However, providers will call patients or caregivers before arrival.

Yes. We coordinate mobile laboratory services and in-home diagnostic testing whenever appropriate.
CCM is a Medicare-covered program designed to help patients with multiple chronic conditions receive ongoing support between visits. This may include medication coordination, care planning, specialist coordination, blood work and urinalysis orders, and proactive follow-up.
Yes. Our providers can prescribe and manage medications when clinically appropriate. Certain controlled substances may require our palliative care providers involvement or outpatient specialist.
Absolutely. Family members, caregivers, and Medical Power of Attorneys (MPOAs) are encouraged to participate in care discussions when authorized by the patient. However, we do ask that one spokesperson be assigned by families to communicate questions and concerns with our staff and providers.
All required medical and consent forms must be completed before services can begin. If the patient cannot sign, documentation from the authorized MPOA or legal representative is required.
Patients, MPOA, or caregivers can complete enrollment forms online through our website by clicking "Enroll Now" or contact our office for assistance with the intake process.
Once all required paperwork and insurance information are received, the enrollment process typically takes approximately 7–14 business days to be scheduled.
