Nicole Vojacek, RN

I am a seasoned Registered Nurse with 18 years of comprehensive clinical experience, with a primary focus in trauma and surgical nursing. Throughout my career, I have had the opportunity to serve in diverse care environments, including a rewarding chapter in travel nursing, where I provided critical expertise and adaptable care across various healthcare settings.

In addition, I have served as a liaison for a skilled nursing organization, where I facilitated smooth transitions for patients entering Subacute Rehabilitation. In this role, I was able to combine clinical insight with a strong commitment to patient advocacy, ensuring that each individual received appropriate placement and comprehensive support for their recovery journey.

Today, I am proud to serve with Onsite Medical House Calls as part of a collaborative team of Nurse Practitioners dedicated to advancing the model of home-based primary care. Together, we bring high-quality medical services directly to our patients where they live—bridging gaps in access and providing personalized, patient-centered care to some of the most vulnerable members of our community.

My passion remains rooted in delivering compassionate, effective care while continually adapting to meet the evolving needs of our patients and the healthcare landscape.
